Forms for parents
When your son joins the club, there are a number of forms we need you to fill in. You can download them below in PDF format. If you aren’t in a position to download these or print them off, then let us know and we can send you paper copies. The membership details need only be filled in once. The medical form likewise, although we’d appreciate being notified of any change in the situation. The consent form can be filled in per trip, per year or for a range of years: just specify at the top of the form when you return it.
